Registration

 

Please click here to see the "Know Your Rights" Guidance available on the New York State Education Department website including a list of documents you may use to prove that you are a resident of our district and entitled to register in our schools.  The District will consider all forms of proof of residency and age listed in the “Know Your Rights” Guidance.  Please contact the District Registrar with any questions about acceptable proofs of residency or age.

 

All children who reside in the Cold Spring Harbor School District with their parent/guardian are entitled to attend our schools starting September 1 of the school year in which they turn 5 years of age by December 1.  There are three steps to the process of registration.

 

Step 1:   Please provide us with information about you and your child

or

Click here to submit an application for registration as a new family. 

Upon completion of Step 1, we will be sending an email to you with links to upload all of the documents we need to complete your child’s registration including proof of residency, proof of age, your child’s health and previous school records.  The documents we need are outlined in Step 2.  You should collect or complete the documents and save them in a folder.

Step 2:   Please download / collect / complete the following forms

Proof of Residency which can be two or more of the documents outlined in the NYSED “Know Your Rights” document.

If you are renting your residence, your proof of residence will be satisfied by completing the following two forms:

If you are under contract to purchase a home but do not currently have the documentation required, please click here to review the Future Resident Policy (5150-R) of the Cold Spring Harbor School District.

Parent Photo ID such as current Passport or Driver’s License.

Child’s Proof of Age such as your child’s birth certificate or passport.

Student’s  Health  Pupil’s Health History  Form – completed by parent 

NYS Health Certificate (Immunization) – completed & signed by a physician  

Physical Form (Completed and signed by physician) – NYS Health Examination  

Dental Health Certificate (completed and signed by dentist)

Home Language Questionnaire (completed and signed by parent).  Note: this questionnaire is available in many languages on the NYSED website.

For students entering Grades 1-12, we will need a copy of your child’s school records.  Please print the appropriate form below, fill in the school your child currently attends, their name, and the grade they will be entering in Cold Spring Harbor SD, and sign the bottom.  Send the signed form back to the registrar, and we will forward the form to your child’s current school.

Step 3:   Bring your folder of documents to an in-person meeting with our registrar so that we can confirm the accuracy of all the information we have!
