Prior to the First Day of School:
7th, 9th 11th Grade Physicals:
New York State Law requires all 7th, 9th /7 11th graders and new entrants to have a physical examination by a New York State licensed physician. The physical must be completed within one year of the start of the upcoming school year. ONLY the approved NYSED form will be accepted. Click on the links below to download/print the medical forms. You may send the physical form to the health office or fax it @ 631-692-8016.
The physical must include height, weight, blood pressure, vision, hearing and scoliosis screening.
2023-2024 Immunization Requirements:
Students entering 7th grade & 11th are required by New York State to have the Meningococcal Vaccine Please submit the updated vaccine record as soon as possible. Failure to provide documentation will result in your child being excluded from school.
If your child has an ALLERGY, ASTHMA, OR OTHER MEDICAL CONDITION, please contact the Health Office to discuss with the nurse.
Communicable/ Contagious Diseases:
Parents are specifically requested to report all cases of communicable/contagious diseases to the school nurse, like strep, flu, etc.
If your child arrives after 7:45 am, they must enter through the main office and go to either the Junior High or High School Attendance desk for a late pass.
If vision and hearing is not completed by your private MD, the school nurse will complete the screening.
7th & 9th Grade Screening: New York State Education Law requires all 7th grade girls & 9th grade boys be screened for scoliosis. If no documentation of screening is received by the school nurse, the screening will be completed by the nurse.
As of September 2009, all school districts are mandated to participate in the survey by the New York State Department of Health. We will be reporting to the New York State Department of Health information about our students’ weight status groups. Only summary information is sent. No names and no information about individual students are sent. However, you may choose to have your child’s information excluded from this survey report.
Medication in School:
ALL MEDICATIONS MUST BE REGISTERED WITH THE SCHOOL NURSE.
NO STUDENT will be permitted to carry or take any medication on his or her own (Including medications such as Tylenol, cough drops, or other prescriptions or over the counter medications), unless a doctor’s order and parent permission in provided to the Health Office. A completed "“Authorization for Administration of Medication” or “Self Carry” form must be completed by your family physician and submitted to the school nurse for any medication needed by your child during the school day.
Medication in school: Any prescription or over the counter medication taken during the school day must be administered by the school nurse. Please download the appropriate form by clicking on the “Authorization for Administration of Medication” link. The parent and doctor must complete the form and return it to the school nurse. All medications must be in a pharmacy bottle with the name of the medication, dosage, doctor and student name clearly labeled. If a parent and doctor request permission for a student to self-administer a medication, you must download and complete the additional “Self Medication Release form” Controlled substances and certain other medications may not be allowed to be carried by the student, please check with the school nurse.
Medications on Field Trips: Please be aware that no medication will be sent by the school Health Office on a field trip with your child unless you speak with the Nurse in advance of the trip.
On overnight field trips where students will be taking medications not normally taken during the school day, Doctors orders and parent permission slips must be on file in the Health Office. This includes prescription and over the counter medications. A packet will be sent home before each field trip.
Medically Excused from Physical Activity
When a physician excuses a student from physical activity, the student will not participate in gym or sports activities.
Registration for Sports Clearance will be done online on FamilyID. Sports clearances & an Interval Health History for Athletics are required prior to every season for which a student plans to participate. A physical dated within one year of the start of the sport must be provided for clearance.
